2023 Midlakes Forms

To be permitted to enter the pool this Season, the following forms must be completed for all swimmers:

  1. Midlakes Concussion Form
  2. Midlakes Hold Harmless Form
  3. Midlakes SafeSport Swim & Dive Program Agreement
  4. Safesport Training

HOA & Guest Members 4/1 – 4/30

Registration for HOA and Guest Members will be open from 4/1 – 4/30 via our Team Unify website Kingsgate Royals Swim Team : (teamunify.com).

NOTE

  • Returning “Swimmer Only” families will receive communication from the Kingsgate Royals Registrar late April. Registration is open 5/1 – 5/8.
  • New “Swimmer Only” families will receive communication from the Kingsgate Royals Registrar 5/9 onwards, subject to availability per age group. See section below for more information.

New Member Registration 5/9 – 5/12

New “Swimmer Only” members are accepted based on specific age group needs.  If your child’s age group is full, your swimmer will be put on a wait list.  To have your family added to the wait list, please email the Kingsgate Royals Registrar the following information:

  • Parent Contact: First & Last Name, Phone & Email
  • For Each Child: First & Last Name, Date of Birth & Gender

If you are on the waitlist, please do not register until you receive emailed confirmation from the Kingsgate Royals Registrar that we have availability for your swimmer to join our team.

Spring Information Meetings (via Zoom)

Returning & Prospective HOA Swim Families + Returning Swimmer Only Families: Q&A Zoom Call Tuesday 4/25 @ 8.00pm.  We will announce the coaching staff, registration details and answer any questions from the team.  Please contact Kingsgate Royals Registrar if you did not receive the meeting link and passcode.

Waitlist & New Team Families: Q&A Zoom meeting will be held in May. Information for those meetings will be sent separately to those respective groups.
